Gynecological laparoscopy is one of the most common surgical procedures worldwide, but evidence regarding the incidence and prevention strategies of surgical site infections in patients undergoing gynecological laparoscopic surgery is limited. Recently, researchers compared the effects of three skin preparation solutions used in gynecological laparoscopic surgery on the incidence of port-site infections, organ or space infections, and any type of surgical site infection.
Sixty-sixty-one patients aged 18 years or older with non-malignant gynecological conditions participated in the study. Patients underwent selective laparoscopic surgery and were randomly assigned to have their skin cleaned with chlorhexidine ethanol solution (n=220), povidone-iodine ethanol solution (n=220), or povidone-iodine aqueous solution (n=221) before surgery. Follow-up was conducted at 1 and 4 weeks postoperatively. The primary outcome was incision site infection at 30 days postoperatively. Secondary outcomes included organ or space infection and surgical site infection of any type.
Of the 661 patients, 640 (96.8%; mean age, 36.2 years) underwent postoperative follow-up. The overall surgical site infection rate was 10.2% (65/640), the organ or space infection rate was 6.6% (42/640), and the surgical site infection rate was 16.3% (104/640). The odds ratio for surgical site infection was 1.13 in the chlorhexidine ethanol solution group compared to the povidone-iodine aqueous solution group, 1.34 in the chlorhexidine ethanol solution group compared to the povidone-iodine alcohol solution group, and 1.19 in the povidone-iodine aqueous solution group compared to the alcohol solution group.
Studies suggest that surgical site infections following gynecological laparoscopic surgery are more common than expected. There was no significant difference in reducing infection rates between chlorhexidine ethanol solution, povidone-iodine ethanol solution, and povidone-iodine aqueous solution.
